Transitioning from a
Skilled Nursing Facility
Moving from a nursing home back home can take a long time, sometimes many months or even a whole year, depending on how much help someone needs and if there's money to pay for it. It's really important to keep trying, to speak up for what you need, and to work together with the nursing home and other groups. This helps make sure you can successfully go back to living in your community. Many independent living groups, like LEAP, have Nursing Home Transition Programs to help people with spinal cord injuries move back to their communities.

Going home from the inpatient rehabilitation center is exciting, but it can also feel like a lot. To make sure your return is safe and easy, it helps to be ready for the changes ahead. Get Up Stand Up to Cure Paralysis has a checklist that lists the important things that people with SCI and their caregivers should know and do before leaving the rehabilitation center.
