Picking a special place like a skilled nursing facility (SNF) or a long-term acute care facility (LTAC) can be hard, especially if you don't have much time. It's a good idea to talk with the hospital team right away about where your loved one will go after they leave the hospital. Planning early helps you find a place that gives really good care.

 

You should also check if your health insurance will pay for them to stay there. It's smart to have more than one place in mind, because the first one you pick might not have an opening. Make sure to tell the hospital staff which places are your top choices.
